The Application of Intentional Replantation in the Treatment of Subgingival Root Caries : A Case Report

Abstract
I. Introduction
Mesially angulated impacted third molar can induce subgingival root caries distally in mandibular second molar. Distal subgingival root caries in mandibular molar is hard to treatment cause of limited access. In many cases, it may be the reason for the extraction of the second molar. In such case, intentional replantation and surgical repair of second molar can be a treatment option to save the tooth.

II. Objective
To describe the intentional replantation & surgical repair as a treatment option for andibular second molar with distal root caries.

III. Case Presentation
1. Sex/age: M/40
2. Chief Complaint(C.C): Spontaneous Pain, mobility on #37
3. Past Dental History(PDH): Surgical extraction of impacted #38, 10 months ago
4. Present Illness(P.I): #37 per(+), mob(++), bite(+), probing depth>10mm
5. Impression: #37 irreversible pulpitis with distal subgingival root caries(due to impacted #38)
6. Tx.plan: #37 Root canal treatment & intentional replantion(surgical repair)

IV. Conclusion
Subgingival root caries in mandibular second molar is a challenge to dentists. It is difficult to access & treatment even if flap is reflected. Difficult access to the defect is the one of the indication of intentional replantation. This case report describes and discusses the surgical repair by intentional replantation as a treatment option for subgingival root caries in mandibular second molar. Definite isolation & sealing without leakage are the important factors for repair of root defects, which makes intentional replantation help improve the prognosis of such cases.
